Reinstatement
of Registration
A
Certificate Of Qualification that has been expired for more than
one year may be restored upon the payment of the required fees
and penalties. The total of these fees and penalties may not exceed
$1,000. In addition, non-resident applicants for reinstatement
must provide proof of current registration in good standing in
the state or jurisdiction in which his practice or residence is
located. The applicant may be required to appear before the board
in consideration of the application. The board may require the
applicant to complete additional requirements and provide evidence
of continuing education. Applications that are denied will be
given an opportunity for a hearing. Although not required, it
is helpful to include a cover letter with your application. The
letter should provide a brief explanation for the lapsed license
along with information on your current licenses, continuing education,
etc.
