Change of Address/Name

Registrants are responsible for keeping their contact information current with the Board.

Use the Change of Address/Name form to notify Board office of a change in contact information. You can email, fax or mail the form to the Board.

Licensees who do not notify the Board of mailing address changes will not receive a renewal notice. It is the licensee’s responsibility to ensure their license does not lapse due to the wrong address.
 
Email address change
 
Most Board communication is via email. Please keep a current, personal (non-business) email address on file with the Board.
 
ARE Candidates
 
ARE candidates must update contact information with the Board as well as Prometric. Prometric does not forward changes in contact information.
 
If the mailing address is not up to date with the Board, exam scores will be sent to the wrong address.
 
ARE candidates must also notify NCARB of their new address.
 
LARE Candidates
 
If you are an LARE candidate with an application on file at the Board office, please let the Board know of any change of address.
 
Candidates are also responsible for notifying CLARB, or their exam scores will be sent to the wrong address.
 
CLARB/NCARB Certificate Holders
 
Persons holding a CLARB or NCARB certificate must notify those organizations of any change of address, in addition to the Board. The Board is unable to notify CLARB/NCARB, nor do they notify the Board.
 
Individual Name Change
 
The easiest way to report an individual name change is to email the Board with the change. You can also fax or mail your change.
 
You must include a copy of the legal document (marriage license, court order, etc.). The document can be accepted via email.
 
Firm Address Change
 
Firms are responsible for keeping their addresses current. Use the Change of Address form to notify Board office. You can email, fax or mail the form to the Board.
 
Firm Email Address
 
An email address is required for all firms. Renewal notices are sent via email. Certificates are sent to firms via email and must be publicly displayed.
 
Firm Name Change
 
A change in a firm’s name requires a new application in order to maintain a Certificate of Authorization. For a New Certificate of Authorization application, go to the Forms page.
 
Deceased Persons
 
Please notify the Board in writing of the death of an exam candidate or licensee. The Board often does not learn of a death until renewal notices are received by a family member. Members of the profession or the public may also notify the Board; however, this information is subject to verification.
